New York State Public 健康 Law 2165 要求所有学生 enrolled at institutions of higher education ARE REQUIRED to provide evidence of vaccination against measles, 流行性腮腺炎, 和风疹, if born on or after January 1, 1957. For more information about MMR requirements please review the New York State Public 健康 Law 免疫接种要求.
Immunity can be demonstrated in 一个 下列方式之一:
- Documentation of 2 MMR vaccines (measles, 流行性腮腺炎, rubella)
- Documentation of measles vaccination (2 doses), 流行性腮腺炎 (1 dose), rubella (1 dose)
- Titer tests (blood tests) showing immunity to measles, 流行性腮腺炎, 和风疹
New York State Public 健康 Law 2167 要求所有学生, 不论年龄大小, to provide proof of meningococcal vaccine (dated within 5 years) or a signed declination statement. For more information about meningitis vaccination requirements please visit the New York State Department of 健康 网站.
Per New York State Public 健康 Law: No institution should permit any student to attend the institution in excess of 30 days without complying.

HOW TO SUBMIT VACCINATION RECORDS
All health information and forms must be completed and/or uploaded to the 学生健康门户网站 before the first day of classes or moving onto campus. Many of these forms are available for electronic completion through the portal. Electronic submission through the portal is the preferred method to receive records as it allows for secure and faster processing, however records may also be faxed to the 健康中心 from a physician's office or former school/university. *转学: Please be aware that immunization records are not forwarded to the University with your academic transcript. You will need to contact your previous college to request immunization records be sent.
Acceptable proof of immunization records:
- Certificate from a health care provider

- Cumulative health record from a previous school

If you are a former student, looking for your immunization records, please log in to the 学生健康门户网站 to see if you're able to access a printout of the required vaccinations your provided to use.
If you are not able to access these through the 学生健康门户网站, you may contact the 健康中心 or complete the 免疫申请表格. An uploaded copy of your photo identification is required to complete the request. Please note: New York State law requires us to maintain medical records for six years from the date of last visit.
